Prior to completing and submitting this form:

1. Carefully review the online parish calendar www.smarymag.org/calendar so that you don't request a space that is already reserved,    
2. Complete the following information before submitting your request for a meeting or event.

We will do our best to accommodate all requests, but keep in mind that all requests are subject to availability and the approval of the Pastor.
